The Student Behavioral Health and Personal Safety Intervention Review process is designed to provide support to school staff who have already worked as a school team to provide multi-tiered systems of support and are seeking additional support, consultation, and resources in the areas of suicide prevention and behavioral health for a specific student or situation.

NOTE: If IMMINENT DANGER exists, call law enforcement and follow your district’s safety guidelines.

In order to maintain confidentiality, please do not list student names on this form.
